Please Help: Donation to P.M.Relief Fund-PAN & Address of Fund

Salaried employee donated fund for victims of Kedarnath Disaster to P.M.Relief Fund Lucknow.Which is available for 80G deduction but problem is that no employee know the PAN and address of fund how take benefit in filing ITR 1.
