Please Help: AFFIXTURE REPORT

receaved a notice under sec 143(2) With affixture report stating that AO has fixed the case,,,,, but in the whole affixture neither the date of case was mentioned nor the place... my question is wheather it is a valid notice with affixture or not & in what way i should reply to this

As your case, on the basis of your PAN has been selected for secrutiny, it is better to meet the AO personally and ask the date and required details for submission.
