please guide whether condonation letter is to be written by the principle or by student , on plain paper or on letter head of firm, to be signed by student or principle.and what is to write in it..please suggest any format.

Dear Anju,
Condonation letter is to be written on the letter head of the firm and it to be signed by principal.
Further, there is no set format of the same and one just has to give reasons for delay in submission of the form.
