15 Authorization letter in favour of certificate applicant on the organization’s letterhead  

     as per format specified in the form

16 True copy attested by partner/director/CS of Last ROI/income statement certified by  

     CA /Annual report/ Latest organization’s bank detail from bank

17 2 Photographs of applicant person
